A member of our Youth Services Team will attempt to reach out to the referral within 2 business days.

Need to refer a YOUTH participant to our prime for life classes?

Once you fill out this form, you’re welcome to immediately schedule them into a class in the second section below. If you do not schedule them immediately, please provide them with a signup card so they can sign up for a class at home.

Ready to sign up for a class?

Please select an upcoming class day/time that works for the participant. The participant’s guardian will receive a confirmation email immediately after sign up and two reminder emails prior to the start of the class.
